Enrollment process - When the need for adult day health services is made known to Happy Valley Day Facility staff, an appointment is scheduled for a Health, Social and Activity Assessment that is conducted by The Happy Valley Day Facility's staff.  This assessment, with input from the client and their family/caregiver, is utilized to develop an individualized plan of care for the client.  The plan of care reflects the client’s identified needs and the services to be provided while attending any of the 2 Happy Valley Day Facility Facilities.
